Are you covered?

The best way to know what you are covered for is to call the number on the back of your card and ask these key questions:

  • Do I have routine vision on my plan? If yes, what is the name of the routine vision? Is it the same as the major medical or does it go under a different name?
  • Do I have a copay/deductible/coinsurance that applies to the routine vision?
  • Do I have hardware coverage under my routine vision? If yes, what dollar amount does my plan cover for hardware? (Hardware coverage means they pay for glasses, contact lenses and contact lens fittings/evaluations)
  • How often do I get these benefits for routine vision? Every 12 months? Every 24 months?

If you do not have routine vision and we can bill your visit with a medical diagnosis, we will, but you will also have to pay the refraction portion of your exam because if you do not have routine vision your insurance will not pay for that.
